Correct epinephrine dosage
02/28/11 at 10:28:49

I'm trying to find the correct dosage of epi in an emergency setting (without an epi-pen).  Can anyone say if this is correct?

Adult

Initial:

0.01 mL/kg IM/SC of 1:1000 solution; not to exceed 0.5 mL of 1:1000 solution (0.5 mg); administer a fraction of the total dose (0.1-0.2 mL) at the site of antigenic exposure, if accessible

Severe anaphylactic reactions (eg, laryngeal edema, respiratory failure, shock):

10 mL of 1:100,000 dilution of aqueous epinephrine IV over 10 min

If there is no improvement, establish a continuous infusion, starting at 1 mcg/min of 4 mcg/mL; increase to 4 mcg/min prn

Would appreciate any comments.  Thanks

Hi Joan ,

I have 0.3ml of 1/1000 strength IM ( intermuscularly ) initally .

for larangeal odeama , shock, respiratory distress .

Then repeated after 5-10 minutes , if i remain symptomatic , then onto infussion .

The epi guidelines are
300mcg / 0.3 ml ( 1/1000 ) for adults 30-60kg ( adult epi pen dosage )

I weigh 100kg , so I often need 0.5 - 500mcg to control a shock .

hugs
Jose
